<title>Microbiological diagnostics in periodontics: biological significance and clinical validity</title>
<description>
For over a century the medical profession has employed clinical microbiology as a tool in the diagnosis and treatment planning of infectious diseases. The identification of etiologic agents helps select the optimal drug therapy to support the patient in overcoming an infectious disease. Clinical microbiology in dentistry is used in cariology, implant dentistry, and periodontics. Recently, microbiology as a diagnostic tool in periodontics was evaluated, with the emphasis on sampling methods and different techniques to detect and quantify target bacteria
(29). The present article discusses the rationale for applying clinical periodontal microbiology in the treatment of severe types of destructive periodontal disease. Microbiological analysis is useful when the information has the potential to direct clinicians towards more effective treatment strategies. The concepts presented here are based on the periodontal literature as well as experience in the field of clinical microbiology as it has existed in the Netherlands for over 15 years. The scientific literature will be used with the proviso that data can be interpreted in different ways and may sometimes result in opposite conclusions. In addition, the scientific basis for a rational use of clinical microbiology in periodontics is still incomplete. However, it is the opinion of these authors that the available information is sufficient to support the use of clinical microbiology in periodontal diagnosis and treatment planning. Since the use of microbial testing in periodontics is to a large, but not exclusive, extent related to the use of antibiotics, this paper will mostly focus on that relationship. 

The discussion in this paper is based on the following views:
• periodontitis is a collection of etiologically different diseases;
• there is growing evidence that some periodontal pathogens have characteristics of exogenous microorganisms;
• microbiological testing can help select patients who are likely to benefit from systemic antimicrobial therapy;
• clinical microbiology in periodontics can contribute to cost-effective treatment.

Toch is het mogelijk op een voorspelbare manier juist gevormde contacten te maken. Bij de verwerking van composiet moet de tandarts rekening houden met specifieke materiaalkenmerken en dus de keuze van hulpmiddelen als matrixsystemen daarop afstemmen. <title>The effect of box preparation on the strength of glass fiber–reinforced composite inlay-retained fixed partial dentures</title>
<description>
Statement of problem. Nonstandardized box dimensions for inlay-retained fixed partial dentures (FPDs)
may result in uneven distribution of the forces on the connector region of such restorations.
Purpose. The objective of this in vitro study was to evaluate the effect of box dimensions on the initial and final
failure strength of inlay-retained fiber-reinforced composite (FRC) FPDs.
Material and methods. Twenty-one inlay-retained FPDs were prepared using FRC (everStick) frameworks
with unidirectional fiber reinforcement between mandibular first premolars and first molars. Boxes were
prepared using conventional inlay burs (Cerinlay), and small and large ultrasonic tips (SONICSYS approx). Box
dimensions were measured after preparation with a digital micrometer. All restorations were subjected to
thermal cycling (6000 cycles, 5_C-55_C). Fracture testing was performed in a universal testing machine (1 mm/
min). Acoustic emission signals were monitored during loading of the specimens. Initial and final fracture
strength values (2-way ANOVA, Bonferroni post hoc tests, a=.05) and failure types (Fisher exact test) were
statistically compared for each group.
Results. Significant differences (P=.0146 and P=.0086) were observed between the groups in the dimensions
of the boxes prepared using conventional burs buccolingually (2.8-3.0 mm in molars, 3.1-4.3 mm in premolars)
and the small size (2.5-2.9, 2.9-3.8 mm) or large size (2.6-3.8, 3.2-4.9 mm) ultrasonic tips for the premolars
and the molars, respectively. No significant differences were found at the initial and final failures between the
conventionally prepared group (842 6 267 N, 1161 6 428 N) and those prepared with either small (1088 6
381 N, 1320 6 380 N) or large ultrasonic tips (1070 6 280 N, 1557 6 321 N), respectively. The failure
analysis demonstrated no significant difference in failure types but predominant delamination of the veneering
resin (85%) in all experimental groups. According to acoustic emission tests, a higher energy level was required
for final failure of the FRC FPDs with boxes finished using small ultrasonic tips.
Conclusion. Standardized box dimensions showed no significant effect on fracture strength at either initial or
final failure of the fiber-reinforced FPDs. The FRC FPDs with boxes refined with small ultrasonic burs required
a greater energy level before failure. The type of failure observed after the fracture tests was primarily
delamination of the veneering resin.

<title>Effect of three surface conditioning methods to improve bond strength of particulate filler resin composites</title>
<description>
The use of resin-based composite materials in operative dentistry is increasing, including
applications in stress-bearing areas. However, composite restorations, in common with all restorations, suffer from deterioration and degradation in clinical service. Durable repair alternatives by layering a new composite onto such failed composite restorations, will eliminate unnecessary loss of tooth tissue and repeated insults to the pulp. The objective of this study was to evaluate the effect of three surface conditioning methods on the repair bond strength of a particulate filler resin-composite (PFC) to 5 PFC substrates. The specimens were randomly assigned to one of the following surface conditioning methods: (1) Hydrofluoric (HF) acid gel (9.5%) etching, (2) Air-borne particle abrasion (50 μm Al2O3), (3) Silica coating (30 μm SiOx, CoJet_R-Sand). After each conditioning method, a silane coupling agent was applied. Adhesive resin was then applied in a thin layer and light polymerized. The low-viscosity diacrylate resin composite was bonded to the conditioned substrates in polyethylene molds. All specimens were tested in dry and thermocycled (6.000, 5–55 ◦C, 30 s) conditions. One-way ANOVA showed significant influence of the surface conditioning methods (p &lt;0.001), and the PFC types (p &lt; 0.0001) on the shear bond strength values. Significant differences were observed in bond strength values between the acid etched specimens (5.7–14.3 MPa) and those treated with either air-borne particle abrasion (13.0–22.5 MPa) or silica coating (25.5–41.8 MPa) in dry conditions (ANOVA, p &lt;0.001). After thermocycling, the silica coating process resulted in the highest bond values in all material groups (17.2–30.3 MPa).

<title>Masticatory function in patients with an extremely resorbed mandible restored with mandibular implant-retained overdentures: comparison of three types of treatment protocols</title>

The objective of the present study was to analyse the effects of implant supported overdentures on masticatory function in patients with an extremely resorbed mandible, and to compare the masticatory function in these patients using three
differing types of implant treatment protocols. The mandibular overdentures were retained by a transmandibular implant, by four endosseous implants following augmentation of the mandible, and by four short endosseous implants, respectively. Sixty patients (50 women, 10 men, mean age 59Æ4 years) were randomly allocated to one of the three  treatment groups. Masticatory function was assessed before and after treatment using a questionnaire, a masticatory  performance test, and a structured interview. The patient-based masticatory function improved significantly. Concerning these parameters there were no significant differences between the three groups before and after treatment. A significant difference existed between the three groups for the laboratory-assessed masticatory function before treatment, but after treatment this difference was no longer significant. From this study it can be concluded that patients with an extremely resorbed mandible and functional complaints of their lower denture report significant improvement
in masticatory function after implant-overdenture treatment. Differences in masticatory function between the three studied modalities were not significant after treatment.

<title>Mandibular overdentures supported by two or four endosseous implants : A 5-year prospective study</title>

Objective: The aim of this 5-year prospective comparative study was to evaluate treatment outcome (survival rate, condition of hard and soft peri-implant tissues, patient satisfaction, prosthetic and surgical aftercare) of mandibular overdentures supported by two or four implants.
Material and methods: Sixty edentulous patients with a mandibular height between 12 and 18mm participated. Thirty patients were treated with an overdenture supported by two IMZ implants (group A) and 30 patients were treated with an overdenture supported by four IMZ implants (group B). Standardised clinical and radiographic parameters were evaluated 6 weeks after completion of the prosthetic treatment and after 1, 2, 3, 4 and 5 years of functional loading. Prosthetic and surgical aftercare was scored during the evaluation period.
Results: One implant was lost (group A) during the healing period. There were no significant differences with regard to any of the studied clinical or radiographic parameters of the peri-implant tissues between the groups. None of the patients reported sensory disturbances in the lip or chin region. No differences in satisfaction were observed between the groups. With regard to aftercare, there was a tendency of a greater need of prosthetic interventions in group A, while correction of soft-tissue problems was restricted to patients of group B.
Conclusion: There is no difference in clinical and radiographical state of patients treated with an overdenture on two or four implants during a 5-year evaluation period. Patients of both groups were as satisfied with their overdentures.
